Full time salary: £26,395 - £26,409 (pro rata)

The Role
As a member of the Linking Leeds team you will make a real difference to the lives of others. You will be a keen advocate of the benefits of supporting people with health and social care needs to access support in their community by linking them to a network of groups and activities; enabling them to have improved health and wellbeing. You will also work alongside health and social care services to support individuals.
