Join Our Team as a Wellbeing Co-ordinator!

Do you enjoy supporting people to improve their health and wellbeing by connecting them with their communities?

As a Wellbeing Coordinator, you’ll play a key role in Linking Leeds – a service with over a decade of experience in Social Prescribing. You’ll support individuals to access non-medical support by completing holistic assessments, co-creating personal Wellbeing Plans, and linking people to community groups and services that boost mental, physical, and social wellbeing. Working across GP practices, community venues, and sometimes from home, you'll help people build resilience, independence, and a better quality of life.

 

Who we’re looking for:

• Someone with a passion for reducing health inequalities and promoting diversity.
• Excellent interpersonal and communication skills, with the ability to build trusting relationships.
• Experience in delivering person-centred support, including risk assessment and wellbeing planning.
• Someone who’s comfortable working across different locations.
