An exciting opportunity has arisen within Leeds City Council’s, Adults and Health Directorate - Care Delivery Service, and we are seeking 4 x System Flow Coordinators.

About the role

The Care Delivery Service consists of a diverse and varied range of social care services for people requiring support. Working with our partners (including health trusts), we specialise in recovery focussed and strengths-based outcomes, enabling people to live independently for longer.

The post holder will work across the range of Care Delivery Service, adopting the Home First approach to flow through our services that support the wider health and social care system. The post holder will work with people accessing our services, their family carers, and others to coordinate a person’s safe and timely admission into and discharge from, our services. Supported by relevant multi-disciplinary colleagues, you will contribute to a high quality experience from admission through to discharge.

As a System Flow Coordinator, you will bring to the role an:

Job purpose

The System Flow Coordinator will facilitate timely access to, and discharge from Care Delivery Services, taking coordination for the person’s recovery journey and transfer of care.

The post holder will work within the Care Delivery Service and support the Home First approach to system flow. The post holder will coordinate a person’s safe and timely admission to and discharge from our services. Ensuring that the person and their family / carers are provided with a high quality experience from admission through to discharge. Supported by relevant multi-disciplinary individuals and teams, the post holder will contribute to the reduction in delayed transfers of care and a reduced length of stay, supporting the wider Leeds health and social care systems.

Responsibilities 

To complete daily delegated tasks from the registered manager to facilitate timely admission to and 

discharge from  Care Delivery Service. This will include but is not limited to:

  • Coordinate with system partners and external organisations to agree timely admission of people from both community and acute settings.
  • To support in the assessment of a person’s care and support needs on admission and start discharge planning from the earliest opportunity.
  • To undertake an assessment, proportionate to the transfer of care to facilitate a timely discharge.
  • Support the safe admission processes. For building based settings, this has specific responsibility for medicine reconciliation and transcription.
  • Support the service manager to monitor, review and take appropriate action to meet key performance indicators.
  • Highlight as early as possible people who have no reason to reside or access services, ensuring they are discharged in a safe and timely manner.
  • Be an advocate of strengths-based social care and for promotion of approaches to maximise independent living and championing the city-wide Home First ambition.
  • Coordinate referrals to system partners and external organisations such as Homecare Providers, Care Homes, Neighbourhood NHS teams and SkILS Reablement service.
  • Confirming post discharge arrangements (e.g. outpatient appointments) and where appropriate, communicating these to relevant parties.
  • Contacting system partners such as GP’s, Community Services and Care Homes to request/provide additional information and/or inform them of discharge planning.
  • Attend and action appropriate tasks as delegated by the Registered Manager or Deputy Manager following Multi-Disciplinary Team meetings.
  • To update the Multi-Disciplinary Team on the progress of patient discharges and any delegated actions.
  • Ensure systems for tracking and monitoring a person’s recovery journey   are recorded, maintained, updated.
  • Escalate issues that cannot be resolved within agreed timeframes in accordance with agreed service processes including those detailed within the OPEL Framework.
  • To attend family meetings and clinical meetings (e.g. MDT, Best Interest) and provide feedback post-meeting to the Registered Manager or Deputy Manager and appropriate persons involved in the person’s care as necessary.
  • Liaise with the team on a daily basis for an updated position on the person’s progress.
  • To communicate in an appropriate and effective manner to people accessing the service, families, or carers where there may be barriers to communication such as language barriers, hearing impairments or cognition issues.
  • To update a person’s records as appropriate, following the Council’s policies and procedures.
  • Maintain personal competency in the use of electronic care records such as CIS and PPM+
  • To comply with the requirements of all Leeds City Council policies, procedures and staff instructions, including responsibilities under the Health and Safety Policy and Procedures.
  • To actively promote and support Leeds City Council’s policies, best practice and values on diversity and equality of opportunity. 
  • Undertake a range of duties as delegated by the Registered Manager or Deputy Manager, working on own initiative within agreed policies and procedures. This includes but is not limited to the following:-
  • Supporting people who access Care Delivery Service  with daily living tasks including personal care.
  • To orientate and welcome new people , families and carers to the  service and provide them with relevant service information.
  • Confirming discharge with relatives, care homes, carers or community hospitals/ other NHS organisations as appropriate.
  • Confirming transport arrangements with relatives and transport services, booking hospital transport if required (in the absence of the admin team).

Qualifications

Commitment to undertake a Level 3 Adult Care Worker Apprenticeship within 12 months of commencement of the post.

Essential requirements

  • It is essential that the candidate should be able to demonstrate the following criteria. Candidates will only be shortlisted if they can demonstrate that they meet all the essential requirements.

  • Ability to work unsupervised and on a one to one basis with people and demonstrate an ability to work on own initiative on a daily basis.

  • Ability to work positively and contribute to multi agency working arrangements.
  • Ability to complete person centred risk assessments and support plans.
  • Ability to review risk assessment and support plans in a responsive and person centred way. 
  • Ability to assist the manager with medication administration and management within RH17 medication policy and procedure. 
  • Ability to participate in formal reviews including the provider-led annual reviews and participate in multi-agency meetings.
  • Ability to maintain positive relationships with partners. 
  • Ability to demonstrate effective support management and to be able to deputise in the absence of the Manager and Deputy Manager for duties commensurate with the grade of this post.
  • Ability to seek, interpret and evaluate information from a variety of sources.
  • Ability to make decisions using all available sources of information, having regard for the consequences and to demonstrate the use of autonomy in reaching such decisions.
  • Ability to deal with any sensitive issues
  • Ability to undertake formal staff supervision.
  • Ability to work with and develop principles of social inclusion and recovery principles.
  • Ability to work with a variety of electronic communications, data systems and technology.
  • Demonstrate knowledge of current, trends, policies, legislation and any regulatory body requirements, in relation to vulnerable adults within a social care setting.
  • Knowledge of the Care Quality Commissions regulatory functions.
  • Demonstrate an understanding of the importance of confidentiality, data protection and information governance requirements.
  • Demonstrate knowledge of the needs of older people, the concepts and methods of delivering a recovery model and on understanding the needs of people accessing the service.  
  • Demonstrate knowledge of Quality Assurance systems and of methods of securing service improvement.
  • Demonstrate an awareness of the needs associated with vulnerable adults, with complex mental health and social needs.
  • Demonstrate an understanding of equality and diversity issues within Leeds and the local community.
  • Requirement to complete the Care Certificate within 12 weeks of commencement of the post. 
  • Demonstrate functional skills in literacy and numeracy at Level 2 or equivalent.
  • Commitment to undertake a Level 3 Adult Care Worker Apprenticeship within 12 months of commencement of the post.
  • Demonstrate knowledge how to foster and maintain effective partnerships.
  • Demonstrate an understanding of the Local Authority, its function and organisational structures and values.
  • Demonstrate an ability to identify problems and seek appropriate solutions.
  • Demonstrate knowledge of strength based social work and the social model of disability.
  • Experience in a support management role in a health, or social care / community setting

  • Hold a current driving licence.
  • Knowledge of health and social care system flow and discharge pathways
  • Knowledge of a range of assessment approaches and techniques of intervention 
  • Knowledge of LCC policies and procedures, including Health and Safety 
  • Knowledge of dementia and the impact on a person’s health and wellbeing
  • Knowledge of disability issues and rehabilitative approaches
  • Experience of working effectively within multi-disciplinary teams
  • Experience of working in a support management role 
  • Experience of community-based work with vulnerable people
