The Leeds Encouraging Activity in People (LEAP) Programme will expand to support people to manage their health conditions as a part of the new Health and Growth Accelerator. Active Leeds is recruiting two LEAP Activators to deliver a programme to support people to remain in employment or to support people back into employment.
Active Leeds aims to recruit enthusiastic and motivated sport, fitness, or health professionals to join our successful Active Leeds - Health Programmes Team. Working closely with Adults and Health providers and Employment and Skills, the team will deliver several community programmes to increase physical activity & provide healthy lifestyle advice, to support people remain economically active.
Purpose of the Job will be:
Supporting the delivery of LEAP Programme by providing activity consultations within community setting and supporting people over a 12-month period to become active through bespoke personalised programme. The LEAP Programme is aimed at supporting people with health conditions such as high blood pressure (hypertension), diabetes, lung disease, musculoskeletal issues and obesity.
The successful applicant will be working within businesses and communities and provide a menu of physical activity and exercise opportunities for people with health conditions. Working within local leisure centres and community centres, the post requires an outgoing individual with lots of motivation & encouragement to give to people, while been sympathetic when needed.
The role will deliver Active Leeds†Healthy Living Programme, the Escape Pain Programme for people with Musculoskeletal (MSK) problems and activities for people with ongoing fatigue is critical such as Chronic Fatigue Syndrome, Long Covid etc.
Duties include:
- Manage referrals from healthcare professionals or businesses
- Deliver programmes in a range of venues
- Provide bespoke individualised programmes for people managing health conditions
- Promote key health messages to target audiences
- Deliver pre and post health assessments
- Support monitoring and evaluation of the programme
- Provide a menu of local activities and signpost people to them.
Skills include:
- Able to communicate with a wide range of individuals & groups.
- Good IT skills, including accurate data collection
- Health Coaching
- Motivate referrals to become and remain active
You should possess:
- The successful candidate should hold a current Level 3 Fitness Professional Qualification such as an NVQ level 3 or equivalent in Personal Training / Health and Fitness or Exercise Referral Qualification, CPR / first aid certificate, group exercise qualification (a circuit, exercise to music or aqua) are also required. All health & fitness qualifications must be endorsed by CIMSPA.
- Ideally candidates should hold a Level 4 Fitness Qualification. There will be training will be available for candidates to gain level 4 qualifications.
- Candidates should have experience of working in a community development, health, fitness, or sport role, ideally working with people with health conditions.

Job Description
Job Purpose
- To inspire, motivate and support the inactive to become active through engagement with local communities.
- Understanding barriers to participation and working to develop opportunities to address individual needs.
- Responsible for the development, co-ordination, and delivery of LEAP Programme. Â
- Provide support and bespoke advice for people to become active using behavioural change techniques
- Work with a range of local partners to recruit people to the LEAP Programme and signpost to people to suitable physical activity opportunities
Responsibilities for Grade
- Support the development of LEAP Programme via engagement of healthcare professionals to ensure the programme is embedded in communities specifically Health Centre and Community Centres.
- Identify priority groups within areas via the Local Care Partnership and support those groups to become physically active.
- Host LEAP clinics in communities to develop tailored bespoke physical activity plans for people referred by a range of health and social care professional to increased participation.
- Develop links with existing community groups and utilise existing community facilities to support the provision of community physical activity opportunities.
- Inspire and attract new participants through engagement events, taster sessions and meetings at various times including evening and weekend work.
- Promote and signpost to appropriate wider physical activity and health and wellbeing programmes/services including other programme delivered by Active Leeds in order to support increased physical activity and access to wider opportunities.
- Communicate effectively in formal and informal situations with a range of partners and participants by a variety of means.
- Develop and support the use of digital platforms. Supporting groups through social media and mobile technology to support community participants to engage and build connections to support retention and on-going participation in physical activity.Â
- Assist with the development of marketing and promotional materials appropriate to the individuals/groups worked with.
- Ensure appropriate administrative tasks are carried out for all projects as required to effectively monitor, evaluate the LEAP programme. This includes using bespoke software developed to monitor the programme. Â Â
- Deliver physical activity and sport sessions to support increased participation in physical activity of local population priority groups.
- Provide reports and presentations as required on the development of the projects.
- Travel to venues within Leeds areas to meet the needs of the organisation.
- Assist in the identification and undertaking of his/her own training and development requirements in accordance with the appropriate organisation employee development scheme.
- To undertake any other duties which may be assigned from time to time and that are commensurate with the grade of the post.

QualificationsÂ
- Educated to NVQ Level 4 or equivalent, or the experience and ability to work at this level
- CIMSPA Recognised Fitness Instructor Qualifications at Level 3 (Exercise Referral)
- CIMSPA Recognised Fitness Instructor Qualifications at Level 4 (e.g., Cardiac Rehabilitation, Postural Stability, Lower Back Pain) (desirable).
