Feel Good Factor (FGF) is a health and wellbeing organisation based in the ethnically diverse area of Chapeltown. We are looking for an enthusiastic and highly motivated individual to fill the following position:
HEALTH & WELLBEING COACH
28 hours per week over 4 days (35 hours per week FTE)
Salary £27,496 per annum pro rata (£21,997 actual)

Feel Good Factor (FGF) is the host employer for this role, which will be embedded within and work as part of Chapeltown Primary Care Network (PCN). Delivered through a partnership between FGF and the PCN, we are seeking an experienced Health & Wellbeing Coach to join the wider PCN team while being employed and supported by FGF.
The successful candidate will work as an integral member of the Chapeltown PCN multidisciplinary team, alongside GPs, practice staff and other primary care professionals. You will make a real difference to the lives of patients by supporting and empowering them to take proactive steps to improve their physical and mental health, based on what matters most to them. You will deliver both one-to-one and group-based support across PCN practices and community settings in Chapeltown.
As host employer, FGF will provide line management, professional support and access to organisational resources, including office space and team development opportunities.
You will have relevant experience, be an effective communicator and have a passion for excellence in your professional field. Most importantly, you will be committed to improving health outcomes and reducing inequalities within the Chapeltown community.
You will be working across locations within Chapeltown PCN, with access to Feel Good Factor’s offices.
