An exciting opportunity has arisen to recruit three highly skilled, flexible, and innovative people to work in our Family Hubs supporting data collection/recording, quality assurance and analysis from a range of early help services.
Family Hubs are new to Leeds and will play a fundamental part in our early help strategy.
As an outcomes co-ordinator you’ll thrive on building relationships with colleagues and partners to enable improved data collection and recording about work done with children and families through Family Hubs, clusters, schools and children’s centres.
The role will be crucial in helping us ensure we get the best outcomes for young people and families in Leeds.

About the role
As our next outcomes co-ordinator you’ll be based within one of our Family Hubs.
The Family Hubs are multi-disciplinary teams in the East, West and South of Leeds. The seven hubs are a key part of the Leeds early help offer, with a focus on providing ‘more help’ to families when needed.
The Family Hubs provide advice and support to clusters and partners working in localities to ensure seamless, co-ordinated and effective early help support and a 'getting it right first time’ response. They deliver direct one to one work to families, as well as a number of evidence based parenting programmes.
You will work with hub managers and our data team to monitor data quality of work recorded on our case management system, identifying and resolving issues, and offering support and training as appropriate to colleagues to improve recording practice.
You will triage work coming into the hubs, recording and assigning it appropriately.
You will build and maintain relationships with local clusters, schools and children’s centres to support the recording of the early help work they are delivering.
You will contact families after they have been supported by Family Hubs to obtain feedback about their experiences, allowing us both to evidence good work and also look at any areas we could improve.
The role is integral to our early help services as we need to ensure that our services are effective and efficient and cannot do this without accurate and sufficient data.
You will play a key role in collecting, recording, quality assuring and analysing that data through building relationships with colleagues and partners and using your skills, knowledge and technical expertise.

Job purpose
To support Restorative Early Support Teams, Rethink Team or Futures Team to operationalise and deliver effective outcome focussed child and family plans.
Responsibilities
- To work alongside the RES team to assist in the effective and efficient delivery of the defined innovation model
- To conduct structured questionnaires, interviews with young people and families for the purpose of the project
- To arrange, attend, and facilitate meetings, conference calls and workshops, including scheduling, agenda planning, preparation and facilitation.
- To manage databases and spread sheets to enable the analysis of defined outcome measures used in the RES team by inputting and retrieving information from relevant systems eg Goal based outcomes
- To help develop a performance management and quality assurance framework for Early Help.
- To help promote and embed a culture of performance management, quality assurance and continuous improvement across the service.
- To assist with the capture, recording, classification, monitoring and reporting of issues and risks.
- To provide statistical information, that enables the production and presentation of briefings and reports.
- To produce reports for board members as required.
- To deal effectively with internal and external telephone, written and face to face enquiries.
- To undertake general clerical and/or word processing duties, including photocopying and the distribution of material effective measurement of outcomes within RES.
- To establish and maintain effective budgetary records, expenditure details and information relating to purchases and orders relevant to the innovation model
- To ensure good quality customer care standards are provided.
- Other such duties of a similar nature and responsibility which provide the innovation project team with support in order to assist their effectiveness and efficiency.
- To provide and participate in training and development activities to ensure you have up to date knowledge and skills.
- To maintain a high level of confidentiality.
- To work within all relevant policies, procedures and systems established corporately by the Council and, in particular, to support the Council’s Equal Opportunities Policy and Health and Safety Policy.
