Job Title:Admission and Discharge Co Ordinator
Salary: *GBP*27,711 - *GBP*30,060 pro rata plus enhancements
Hours: 18.5 hours (part-time)
Contract: Temporary for 6 months initially.
Location:Dolphin Manor

About the Role
As an Admission and Discharge co ordinator, you will provide essential support to the service and the team, by supporting admission and discharges in the service. Flexibility is key, as you may be required to work across different services.

About You
We are looking for individuals who bring the following to the role:

  • Experience managing a staff team in a care setting.
  • Proficiency in using Microsoft Office applications.
  • Knowledge of strength-based care.
  • Understanding of the needs of older people.
  • Familiarity with CQC and Adult Social Care assurance processes.

Job Description

Job purpose This role is central to maintaining system flow across care services by ensuring timely and safe transitions for individuals entering or exiting care settings. It supports both operational delivery and strategic coordination across multiple stakeholders, including health and social care partners.

Responsibilities

  • Support Admission and Discharge Processes
  • Attend and contribute to MDTs
  • Liaise with Professionals, Families, and Carers
  • Arrange Assessments and Advocacy
  • Maintain Records and Systems
  • Promote System Flow and Accountability.
  • Contribute to Service Improvement

Qualifications

Essential requirements Candidates will only be shortlisted if they can demonstrate that they meet all the following essential requirements.

Level 3 Health and social care or be willing to undertake.
