Leeds Shared Lives is part of Leeds City Councils Care Delivery Services, Adults and Health. We are an in-house provider service offering Respite (Short Breaks) and Day Support to adults with Care and Support needs across the city.
Our service is an alternative to respite in a care home, or Day Opportunities. Individuals requiring care and support, are matched with a Shared Lives Carer who will offer Respite or Day Support from their own homes.
This is an exciting opportunity to join a well-established and respected team within Adult Social Care.
The service is a registered provider with the Care Quality Commission.
To support and promote teamwork, you will be expected to commit to hybrid working. The Team is located at Tribeca House, Roundhay Road, Leeds 7 and our core business hours are 8:30-5 (Mon-Thurs) and 8:30-4:30 (Fridays)
About you
As a Wellbeing Worker, you will bring to the role:
- Good knowledge, experience and a passion towork with vulnerable adults.
- Effective communication skills, both written and verbal, as well as possessing numeracy abilities and being a team player.
- An understanding of relevant legislation such as Care Act 2014 and the Mental Capacity Act (2005) some knowledge of Health and Safety Legislation, as well as an understanding of the requirements of the Health and Social Care Act (2012), its regulations and other relevant legislation and guidance.
- You will be motivated and a critical thinker with the ability to assess the needs of your customers, act as an advocate where appropriate and ensure appropriate risk management and safeguarding
- Strength based approaches and a strong desire to make a difference
About the role
You will be required to carry a caseload of Shared Lives Carers and Customers with the supervision and guidance of the Registered Manager or Social Worker where required, to support the delivery of a Shared Lives Service across the city.
You will have a key role to undertake strength-based assessments and create outcome-focused support plans including, the review of individual plans relating to the assessed needs of service users, their families, and carers.
- You will be expected to contribute to and undertake the screening of new referrals for service users and prospective carers and undertake the matching process of service users with carers.
- You will be responsible for monitoring placements, liaising with Social Workers, Care managers and other professionals where relevant, to ensure best practice and a safe and effective service.
- You will be monitoring the role of Shared Lives Carers on a regular basis, supporting their development to ensure quality in service is maintained.
Successful candidates will also be required to contribute to service development and develop their own performance and skills through CPD (Continuous Professional Development).

Job Description
Job purpose
Under the direction of a Registered Manager and with support and direction from Social Worker colleagues, provide a high quality and comprehensive assessment and support planning service, advising adults with care and support needs. In doing so, promote the principles of wellbeing as described within the Care Act 2014.
The post holder must at all times carry out responsibilities within the City Council’s Policies and Procedures, in particular the Council Policies on Equality and Diversity, Health and Safety.
Responsibilities
- Responsible under the Care Act 2014 for working with a range of service users to develop services and plans that meet their assessed needs.
- To work within a community or hospital based team, providing person centred support in line with the Care Act 2014
- Contribute to the promotion of improved outcomes for individuals, families, carers and groups from diverse and disadvantaged communities
- To undertake and contribute to the review of individual plans relating to the assessed needs of service users, their families and carers.
- Maintain and update case notes and other electronic records, write reports as required within the limits of data protection
- With supervision, to carry a caseload of appropriately allocated cases and undertake assessments of service user needs using a person centred outcomes focused approach.
- Where required, prepare person centred support plans in conjunction with service user, their families and carers.
- Where safeguarding issues arise on the Wellbeing and Recovery Worker caseload, to escalate them to the manager and to assist the Social Worker to investigate and manage risks.
- To ensure that any concerns about practice and procedures are raised with the team manager
- Organise work activities on their caseload and prioritise tasks as required.
- With support to undertake office duty as required.
- To provide general advice about the Council’s charging policy
- Develop and maintain effective working relationships with other professionals within the directorate and partner agencies to ensure an integrated approach to the support planning and safeguarding of service users.
- Be responsible for understanding and applying all appropriate practice standards, policies and procedures.
- Responsible for assessing and working with the client to devise a support package that reflects the assessed needs of the service user, with support from a Social Worker where required.
- Be responsive to feedback and self-reflection around practise.
- Appropriately participate in meetings and relevant forums.
- Work closely with line manager using supervision and appraisal to develop own performance and skills through CPD.
- To use supervision and appraisal to continually develop professionally.
- To comply with the requirements of all Leeds City Council and Directorate policies and procedures and staff instructions, including responsibilities under Health and Safety policy and procedures.
- To actively promote Equality and Diversity throughout the service.
- To undertake any other duties and responsibilities appropriate to the grade and purpose of the job as may be determined by the directorate

Qualifications QCF Level 4 diploma in Adult Care would be desirable.

- Possession of a full current driving licence
